Client Rights and Responsibilities

Client's rights are prominently posted in all Transition Strategies offices.

All clients are made aware of their rights and responsibilities during their
initial appointment. Counselor consultants request that each
client or guardian sign an informed consent acknowledging an understanding of
their rights and responsibilities. Transition Strategies provides all
clients with a copy of their rights upon request.

Clients have the right to be treated with dignity and respect as individuals
who have personal needs, emotions, preferences, and requirements.

Clients have the right to impartial services available without regard to race,
religion, gender, ethnicity, age, disability, or source of payment.

Clients have the right to privacy with regard to provided services and in
pursuit of needs that are addressed by organization staff.

Clients have the right to be fully informed of available services in a
continuum of care and any charges or limitations related to those services.
Clients have the right to know service or assistance options regardless of
whether those services or assistance options are available through the
organization or its parent organization and regardless of the cost of those
services.

Clients have the right to receive information about Transition Strategies
services and the role the organization plays when services are recommended by
the organization, including guidelines used in making referrals.

Clients have the right to refuse service without compromising their access to
other organization's services to the extent permitted by law, and to be
informed of the consequences of refusing services (or treatment).
