All counselor consultants are licensed clinical social workers or licensed clinical psychologists, with experience in working with families in transition.

How are the referral resources selected?
Transition Strategies maintains a comprehensive database of facilities and organizations that provide services frequently used by the elderly. Transition Strategies, by policy, does not accept referral fees or other compensation for referrals. Site visits are made in person to see a facility or program. All referral resources are re-credentialed annually to assure continued compliance with industry standards and Transition Strategies.

If a referral is needed, what information will Transition Strategies provide?
Referrals may be made to a category of support services (e.g., nursing homes, home health agencies) or to specific vendors or providors.
